About your First Visit:
At your child’s first visit, you will receive a patient care package that explains our policies and procedures. You will also complete forms to give us information about your child’s medical history and to give your consent for us to treat your child. These forms may be printed and completed at home to save you time at check in. Simply click on the links below to print the forms, fill them out ahead of time, and bring them with you.

About Follow-up Visits:
Normally, we schedule routine follow-up visits every six months to check the development of your child’s teeth and to clean them. At check in, we will give you a form so that you can provide us with any information that may have changed since your last visit. To save time, print out the form below, fill it out, and bring it with you.
As a parent or guardian, you can play a big role in helping your child feel at ease when visiting the dentist. Fear of the unknown is a big reason children cry. Calmly discuss with your child what he or she can expect to see—people wearing masks and gowns and gloves and a big room with lots of reclining chairs and equipment and bright lights. Let your child know that he or she will lean back in the chair, and people will look inside his or her mouth. The staff will use some instruments to check his or her teeth. Your child may hear whirring noises as the staff cleans teeth. If you can convey your positive attitude and knowledge to your child, he or she will be less frightened.
If your child does cry, please be assured that our staff is trained to help your child through the experience.
About Surgery and Sedation:
Should your child require surgery, Drs. Stokes and Caldwell are fully trained to sedate your child appropriately. Should your child require conscious sedation, Drs. Stokes and Caldwell can perform the procedure in the office. Should your child require general anesthesia, Dr. Stokes and Caldwell are on the medical staff of Cape Fear Valley Medical Center.

We accept all insurances and Medicaid. You may pay for services by cash, check, Visa, Mastercard or Care Credit.
